What do I need?
For minors ages 16 & 17 - at least one parent or guardian must be present to sign and present ID. Minors ages 16 & 17 can use a student ID and social security card if a Driver's License or ID is not available.
DS-11 Application Form
- Form must be completed in black ink prior to your appointment.
- If you make an error, complete a new form. Do not correct.
- Complete lines 1-22. No double-sided pages.
- Do not sign.
Proof of Identity
Bring one of the following (along with a front/back photocopy, not double-sided):
- Valid Driver's License or State ID
- Current Passport
- Government Employee ID
Proof of U.S. Citizenship
If you were born in the U.S. you must submit one of the following:
- Previous U.S. Passport (Required if available). Bring original and a photocopy.
- Certified birth certificate required. Bring original and a photocopy. Must contain the names of the parents.
- If passport is lost, complete DS-64 form.
Passport Photo
The Library does not offer a Passport Photo service. See the list below for local services. The photo has the following requirements:
- 2" x 2" Color photo with a white background and a clear image of your face.
- Must have been taken in the last six months.
- No selfies and take off your eyeglasses for your photo.

Payment #1 to the U.S. Department of State
Check or Money Order only. Paid separately from Payment #2.
Passport Book …. $130
Passport Card …. $30
Payment #2 to the City of Alhambra
Cash, check, or money order (exact change only). Paid separately from Payment #1.
Processing Fee …. $35

Applicants are responsible for reviewing the list of items you need and bringing the required payment types necessary for their appointment.

Please arrive 10 minutes early with your ID, payment, and all required documents filled out prior to your appointment.
Arriving to your appointment late or with incomplete documentation, may forfeit your time slot.
