What do I need?
For a new or renewal passport for a child ages 15 and under, the child, and both parents, or the child’s legal guardian(s) must appear.
DS-11 Application Form
- Form must be completed in black ink prior to your appointment.
- If you make an error, complete a new form. Do not correct.
- Complete lines 1-22.
- Do not sign.
Both Parents/Guardians Present
For non-appearing parents/guardians:
If one parent cannot appear in person – The non-appearing parent may complete and notarize form DS-3053. All DS-3053 forms must be filled out in black ink and must include a legible photocopy of the front and back of the non-appearing parent’s valid driver’s license or government issued ID.

Proof of Parental Relationship
Abstract and hospital birth certificates are not acceptable.
- Certified birth certificate required. (Must contain the names of the parents.)
- Certified Court Order of Guardianship
Proof of U.S. Citizenship
If you were born in the U.S. you must submit one of the following:
- Previous U.S. Passport (Required if available.)
- Certified birth certificate required. (Must contain the names of the parents.)
Passport Photo
The Library does not offer a Passport Photo service. See the list below for local services. The photo has the following requirements:
- 2" x 2" Color photo with a white background and a clear image of your face.
- Must have been taken in the last six months.
- No selfies and take off your eyeglasses for your photo.

Print the Passport Pamphlet with the List of Items You Need
Applicants are responsible for reviewing the list of items you need and bringing the required payment types necessary for their appointment.
Passport processing times fluctuate – please check current processing times here.
Please arrive 10 minutes early with your ID, payment, and all required documents filled out prior to your appointment.
Arriving to your appointment late or with incomplete documentation, may forfeit your time slot.
